Case mismatch between loaded and declared class names

jeeva r → made their first commit to this issue’s fork.

I had rename the file @prudloff.

Renaming the class would be a breaking change.
I think it is less disruptive to rename the file to match the class.

@prudloff, if we rename the file, then we need to change the name on routefile also. I checked that, class rename has doesn't make any site breaking.

Oh right, I didn't see that email_login_otp.routing.yml already uses the OTPForm case.
