Widget fails to load

I've set up this module according to the instructions, but I'm the widget does not load on the form. The field label is there, and this error is present in the console:

Uncaught TypeError: Cannot read properties of undefined (reading 'form_element_settings')
    at location_selector.js?v=10.4.6:14:59
    at location_selector.js?v=10.4.6:359:3

I've tried adjusting the widget settings and that doesn't seem to change the behavior. I've also tried a couple admin themes but no luck there.
