Allow bypass of all outbound URLs

Problem/Motivation

It would be useful to have a way to ignore all outbound URLs on a site if that feature isn't used, rather than having to explicitly specify outbound URL ignore paths.

The attached patch adds a variable to allow sites to bypass adding the changeset_id parameter to outbound URLs entirely.
