Image file directory name hardcoded.

Problem/Motivation

The file_directory setting on the "image" base field is hardcoded to "credentialing_providers". Which 1, seems like a weird name and 2. would be better if you could configure this.

Proposed resolution

Rename to a more meaningful directory, or make it configurable.
