Implement basic versions of the code to rebuild entity access records

Problem/Motivation

Currently there is no way to rebuild the entity access records - for example, when a new module is enabled that implements one of the entity access records hooks.

Implement a first pass of this using the node module as a template. Possibly also define an event that can be used at the end of the process for modules that want to e.g. write an all grant.
