Image can only be replaced one time before "add media" stops working

Important to note
I begun noticing this in 0.x as soon as the placeholder image became available as the current ML selection. So this is before 📌 Media Library dialog styling Active landed. That issue has been a bit of a magnet for being the perceived cause and/or solution to many unrelated Media Library things, so I think it's helpful to take that out of the speculation pool so we can reach the right solution faster.

To reproduce, use the Media Library widget to replace an existing image, remove that image, then try to add another. At this point "Add media" does not work.

When I run into this - if I manually run Drupal.attachBehaviors on a context containing that button, it will again work, so the solution will likely be finding where we are neglecting to call Drupal.attachBehaviors and finding a way to reliably trigger it.
