Improve default formatter for the notification_settings field type

Problem/Motivation

The notification_settings_default field formatter is currently not themeable also the plugin is not defined with the new PHP attributes.

Proposed resolution

Change plugin annotation to attributes and introduce custom theme for the formatter.
