Replace dummy data with actual values from the current order

All committed along with a revision to the JavaScript per opdavies's original work to copy values to the script object from the div template. I also added the JavaScript src URL to the div, varied by environment, and added a setting for the customer ID, which appears to be a static value based on the merchant account.
