autofocus attribute focused on load by Chrome

Chrome, incorrectly, focuses the autofocus attributes added within dialog elements on page load. This is contrary to the spec, which states that focus only happens when the dialog is shown, not on load. This chrome bug does impact usability, so we should remove the attribute.
