Currently, on the "Choose block" dialog, we set the initial focus to the first category. However, accessibility best practices are to focus on the close button on the dialog.
Also, when a block preview is shown, there is no indication on the link that that particular block is being shown, so it's hard to tell which one is being previewed at a glance.
Let's fix those things!
