Title optional

Sometimes a document doesn't have a title. This is for records like a PDF index where the title metadata hasn't been set.

Try to query for a document that doesn't have a title. The GraphQL crashes.

Make title optional and transition the title response to the `title` field instead of the `htmlTitle` field.
