Login.gov docs don't mention AAL/3

The login.gov documentation doesn't mention AAL/3 (anymore?).

This modules can request AAL/3, and it works. Check with the login.gov staff and see what values are really supported.

The documentation no longer mentions a 12-hour forced-reauth at AAL/2. Form text should be updated.
