Disconnect confirm form leads to 404, doesn't disconnect authentication

Problem/Motivation

If incorrect user is used for authenticating with DAM or the site needs to be disconnected for another reason the disconnect button doesn't work.

Running version 1.0.13 and Drupal 10.3.1.

Steps to reproduce

  1. Set up module and connect to DAM using Authorization code (standard) mode
  2. Click the disconnect button in the config settings form
  3. Click the confirmation disconnect button at /acquia-dam/disconnect
  4. Results in a 404 at a page with the URL format /acquia-dam/form_action_p_[ENCODEDSTRING].
  5. The disconnect doesn't take place and there are no errors logged besides the 404.

Proposed resolution

Seem to be a problem with the submit handler which may not be firing at all.

    We've started running into similar problems with other forms and this seems to be related to Site Studio or the theme in place so I'm closing this.
