Issue with Saving Payment Methods

I'm encountering an issue where payment methods cannot be saved. When I add a payment method through /user/5/payment-methods/add, the system indicates that it has been saved upon submitting the form. However, the payment method does not appear on the Payment Methods page for the user. Could someone help troubleshoot this problem?
