Support more than one field & othe field types

Problem/Motivation

Supports just one input field. Often you need to track more than one field.

Proposed resolution

Allow for supporting more than one field and support for select, check boxes, etc.

Data model changes

What would I need to change to get it work with one textfield and 3 select fields?
