Disable "Multiple" checkbox has no effect

Problem/Motivation

Disable the "Multiple" checkbox ("Customer should be able to select multiple add-ons") still allows choosing more than one add-on per product, using checkboxes.

Proposed resolution

Disable the "Multiple" checkbox should change the fronted to form type "radio".
