Remove ctools dependency

The module has a dependency on Ctools but it is only used for the Entity Form Wizard (multistep form).

Historically, memory_limit_policy was build this way because it largely inspired from password_policy approach.

The policy form is not complex enough to require a multistep form and the overhead of maintaining ctools dependency. Even password_policy reviewed the approach and does not need ctools anymore.

We should review the form to remove the dependency to ctools and simplify the entity form.
