When inserting a reduced size image, with setting of my content type to be "Link image" to "Original image", the image shows properly with the expected size, but no link is created.
Using latest Drupal
Content type to be "Link image" to "Original image"
Editor: BBCode (properly setup in the Insert module)
Dev mode: disabled
Cache: cleared
I upload new image, set size to 320x320, insert the image, output is

Thanks for reporting the issue, I can reproduce the problem. Linking to an image style seems to work, but linking to the original image does indeed not work for me either. I have attached a patch that fixes the issue for me.
