commerce_add_to_cart mutation improvements

1. In case no cart exists, a new one is created based on a store id sent in the request header.

3. In case a line item exists with the same variation and custom field content (optional), the configuration of commerce is respected (line item stacking by default)
4. Document the mutation in the README.
