Add explanation of how various scenarios are handled to README

Problem/Motivation

The README could really do to explain how this module reacts to various circumstances, step by step.

e.g.

- upstream changes in shipped config
- deleting local copy of shipped config (it's mentioned, but a list of cases would be clearer)
- changing local copy of shipped config
- other things I've not thought of ;)
