indicator didn't show for admin user until permissions had been saved

Problem/Motivation

I enabled this module, set some values in settings.php (I just copied the sample PHP code from the README), then loaded some pages.

There was no sign of the indicator.

I went to the permissions page, granted the 'See all environment indicators' to all roles, saved, and THEN I got the indicator.

When I removed the permissions from all roles (except the administrator role which is automatic), the indicator stayed visible.

I tried uninstalling and reinstalling the module, but wasn't able to reproduce this.
