Adding a toolbar button outside of a group causes PHP errors

A patch to remove the ability to add toolbar buttons that do not have an associated group and stop using group name variables that may not be defined.

Tested #2 and committed to Editor 7.x-1.x.

Automatically closed - issue fixed for 2 weeks with no activity.
