The

on drupal.org shows an old screenshot that has a free form text field as the first form field. The actual module has a dropdown select field that allows you to select an existing taxonomy to import into.
Please update that screenshot.
Also, add information about:
- if existing terms are kept
- if deduplication occurs during import (existing term with the same name will not be imported/created again)
- if the first line in the CSV is skipped.

Although the documentation of the module is already included in the README of the project, maybe some more information on the module page would be a good idea.
