Stored cookie categories do not exist on first page load

Using eu_cookie_compliance opt-in with categories.

When loading a page for the first time, there are no cookie_agreed_categories. Therefore, the _paq.push(['requireConsent']); _paq.push(['disableCookies']); won't be added to the script. This results in Matomo always setting cookies, even when the visitor did not agree.

Also, the current implementation would set those _paq.push multiple times if a visitor already disagreed with multiple categories.

Raising the priority to Major since this makes the whole module useless.
