Add UI for Tracker and Connector config

Problem/Motivation

Setup of this module currently involves creating migration config that is specific to the given site. This requires fairly substantial knowledge about how Migrate QA is structured, and is a significant barrier to using this module.

Proposed resolution

Add a UI that generates the needed tracker and connector migration configs.
Add a trigger/event/hook that runs the correct migrations to generate trackers and connectors at the right.

Remaining tasks

Add the UI.
Add the config storage mechanism.
Add the trigger/event/hook to fire the correct migrations, and the supporting migration source plugins.

New forms to use to manage tracker and connector config.
