H5P Documentation Tool Goals not working

Problem/Motivation

H5P documentation tool Goals page is not accepting any goals and resulted opigno-tinca-api/relay 500 error. This is being tested on demo site as well. Same results.

Steps to reproduce

Create an activitiy using H5P documentation tool.
Choose Goals Page.
Create a Module.
Add that activity to Module.
Create Training.
Add that Module to the training.
Start the training and open Browser console.
Create a new goal and add more goals
You should be seeing http:///ajax/opigno-tincan-api/relay 500 error in console.
After clicking next to finish the training, at the result page, there are no goals showing.
