Display urls where specific view modes are used

Problem/Motivation

By providing this url, it makes it easier to identify the context in which view modes are being used. This module already logs this data. We just need to make it easily viewable via the UI.

Proposed resolution

Perhaps we should display the number of occurrences in the view mode usage report as a link to a page that displays a list of occurrences, including the url.
