Update theme settings form

The current theme settings for UIKit include a lot of old stuff that Richard set up that aren't there any more. Additionally, I'm going to add a way for theme admins to update/rollback UIKit versions on their own instead of pinning each theme version to a dependency version.

    Removed outdated info from "about" section.

    Added settings to use bundled assets or CDN assets on any version.

    There is currently an option for "custom location" that does nothing right now. It's effectively identical to the "bundled version" option.

    I'm envisioning something similar to the favicon & logo settings where admins can set their own paths.
