Unomi Conditions Settings form should explain the URI setting is relative to the base URL configured elsewhere

The form at /admin/config/system/unomi-conditions just says "Conditions URI".

It's not clear that this is supposed to be a URI relative to the unomi connect URL entered elsewhere.

This should be state in the form element description. It would also be helpful to output the URL as a prefix to the textfield.
