Module cannot be uninstalled

When uninstalling the module, it redirects to a survey to ask me why I'm uninstalling, and regardless of whether I fill it in or skip, it then redirects to a 404 page and the module remains installed.

You should not be doing this. I have to revert to a previous database backup to remove this module.
