Migrate simple_sitemap_page_manager to use simple_sitemap ^4

Problem/Motivation

The simple_sitemap_page_manager v1 was compatible with simple_sitemap 3, the idea is to migrate simple_sitemap_page_manager v2 to simple_sitemap 4.

Proposed resolution

The form helper class from simple_sitemap has changed it's methods names, so the idea is to change the .module file to be compatible with it.
