Exclude Table of Contents

Problem/Motivation

To exclude the table of contents, there is an empty select option that is supposed to mean do not automatically show toc.
This is not the best user experience and does not work for the following situations.

The Chosen Module excludes empty options from the list.
This is also not good for accessibility.
