Don't show disabled content reminders on node edit form.

Problem/Motivation

It can be confusing for editors if they see a content reminder listed on the form when the content reminder has already been sent or disabled.

Proposed resolution

Only show the content reminder if it is enabled. If an editor tries to add a new content reminder using the node form then update the old one that is related to the node and set it as enabled.
