Restrict repetition on whole page (+choose definition field and cache clear on vocabulary changes)

This patch mainly provides the ability to select the behavior of the "repetition" option.
You get 3 options : find all occurrences on page, find only first occurrence in a same text block, or find only first occurrence in the whole page.
It also provides the ability to choose the taxonomy field used for definition.
Finally, it adds a cache tag to clear cache on vocabulary changes.
