Add tracePropagationTargets config for JS and PHP; set baggage header and meta tag

Problem/Motivation

We should support configuring tracePropagationTargets for JS and trace_propagation_targets for PHP, and set the baggage header for HTTP client requests (if request is to a propagation target) and the baggage meta tag for pages. Note that we will need to make sure the transaction name is set to the route path, if available, when generating the baggage (currently it's not set to the route path until after the response is sent, on terminate).
