UI - Request to join modal form styling issue

Problem/Motivation

Request to join modal form styling looks mis-aligned i.e having scrollbar, compared with previous version.

Steps to reproduce

1. Navigate to group stream page
2. Click `Request to join` action button and the form that appears looks mis-aligned having an additional scrollbar
