During the login process, we're seeing this notice in the logs:

Notice: Undefined index: HTTP_REFERER in Drupal\miniorange_saml\Controller\miniorange_samlController->saml_login() (line 109 of ./web/modules/contrib/miniorange_saml/src/Controller/miniorange_samlController.php)

It's not uncommon for the HTTP_REFERER to not be set.
