Unable to add Submit form event to model

Problem/Motivation

I am unable to add either the Submit form or Process form events to a model.

In all attempts, the form to add the event refreshes with all of the textfields emptied and no status message to indicate an issue.

I have tried various combinations of including data in the text fields on this form - from just having a single restriction to entering data for all possible restrictions. In all cases the result is the same.

Steps to reproduce

1. Create a new model
2. From the add event dropdown selector, select “Submit form” and click on “Add event”
3. On the resulting form enter data in 1 or more text fields
4. Attempt to save the event

You should find that the form reloads with all previously entered data removed, and the event has not been added to the model.

Note that also on this form the “Cancel” button has no effect.
