Getting error on any page this form is rendered while on PHP8.1 - Deprecated function: Optional parameter $form declared before required parameter $form_state is implicitly treated as a required parameter in include() (line 571 of /srv/prod-nsf/vendor/composer/ClassLoader.php).
Fix function declaration.
