Fix coding standard violations.

The patch in #6 looks like PHPCBF was used to automatically "fix" violations with no attempt to read or understand the changes that were being made. It introduces half as many new violations - mostly in the form of trailing spaces - as it fixes. But worst of all, a class docblock was deleted that contains a required copyright statement. I was so distrustful of the #6 patch that I decided to ignore it and start a new one beginning with the one from #2.
