Port to Patreon V2

Problem/Motivation

This module is a little outdated and has a hard dependency on a specific Patreon API library version.

Steps to reproduce

If you install the module, most of functionality it will not work.

Proposed resolution

Refactor the code to support the API v2 and Patreon library >= v1.0.0.

Remaining tasks

I'm starting the refactoring on my own and will send some improvements soon.

User interface changes

There will be some user interface changes to accommodate the changes on V2

API changes

There will be some changes to accommodate the new Patreon API v2
