I tested and cannot reproduce this problem on last stable and dev versions.
Could you test this issue on last version of module (8.x-3.6) and close the issue or add more information.Thanks.
I have a transition called "Ready for review", and to that transition, I have defined a notification in the moderation notification module.
Now if I change the name of the transition to "review-ready", I can no longer access the moderation notification module and only get an error message. The module crashes.
