Provide a checkbox to protect all contact forms

MR !16 - Added 'All contact forms' checkbox to enable all contact forms.

The patch works, but I don't like the usability because the "All contact forms" checkbox seems to be an equal of all the other checkboxes and there's not indication that checking that will hide the other forms. Likewise, there's no indication that unchecking that will allow you to select individual contact forms.
