Videos not uploading

I am not sure if what I have is a configuration issue or a code one.

using 7.x-2.0
Jquery 1.9 as default (Applied ie patch)
using services as stated in Drupalize video but now I see I don't need it(?)
took me a while to find the right library as there are several references to the various Github API libraries
Attached 2 images of video upload process

• first shows browse button but no upload / submit button
• second shows video uploading as soon as video is selected during browse phase. It just uploads it without warning / request

There is no progress indicator or completion message. Videos never make it to backlot because there are none displayed in need to approve or transcode progress

Ideas?
