All these years later, why not just update status of these issue.
Hi,
The module seems to do a good job capturing the user information, but the email message seems a little lacking. It would be really nice if the document name(s) could be included in the email. If there are multiple downloads, in order to follow-up it would be helpful to know which items were downloaded.
Also - minor cosmetic point - below the name and email info, the words "Download" and "Cancel" run together as one word - "DownloadCancel". I edited the .js file to add " or " after Download, so it now appears "Download or Cancel".
